At Community Health Plan of Washington (CHPW), our mission is to provide comprehensive health care coverage to those who need it most. Founded in 1992 by Washington's community health centers, we are committed to serving the health needs of the people in our state. Our Services
We offer a range of services to support our members in achieving their overall well-being. Our services include:
- Behavioral Health: We provide access to mental health and substance abuse services to help our members manage their mental health.
- Disease Management: Our team of healthcare professionals works with members to manage chronic diseases such as diabetes, hypertension, and asthma.
- Pharmacy Services: We offer a network of pharmacies where our members can fill prescriptions and receive support from our pharmacists.
- Transitional Care: Our transitional care programs help members transition from hospital to home with the goal of reducing hospital readmissions.
- Case Management: Our case managers work with high-need members to identify and address their unique care needs.
Community-Based Services

Eligibility
We offer coverage for Apple Health (Medicaid), Medicare Advantage, and Individual & Family plans. To be eligible for coverage, individuals must meet specific requirements and live in our service areas.
